Care Management

Care Management is one of the services of which WRAAA is most proud. It is designed for those individuals age 60 and older who need assistance accessing resources so they can maintain independence at home. The Care Manager can assist you in gaining and coordination access to many services: medical, social, educational, and any other service appropriate for individual needs.

Based on your needs, our Information Specialist and Care Manager will begin the process of determining exactly which programs and services can help you. They will assist and explain options for your care.

  • Development of the care plan based on the needs of the consumer identified through a comprehensive assessment.
  • Coordination of social and medical services needed including those available through WRAAA or other non-AAA sources.
  • Communication and coordination with family support.
  • Ongoing care management, monitoring and follow-up

Communicate and link to service providers regarding needed services as part of a care plan (financial, legal, housing, and healthcare).
